Preparing for a medical appointment: a practical checklist

A printable outline to help you arrive with priorities and leave knowing what happens next.

KEEP IN MIND

Two or three priority questions can help give an appointment structure.

Bring useful information

The NHS advises noting important questions, the medicines you take — including vitamins and supplements — and useful details about symptoms. Request an interpreter or communication support in advance if needed. You can ask someone close to you to come along.

An outline to take with you

Our practical worksheet: take a sheet of paper and use the four headings below. You do not need a complete narrative; a few words and dates can start the conversation.

  1. My priority: what I want to discuss first.
  2. My observations: when it started, when it happens and its effect on daily life.
  3. What I use: medicines, supplements and documents to show.
  4. What happens next: the next step, timeframe and contact person.

Leave space under the last heading for notes during the appointment.

Before you leave

Check that you understand the next steps and how any results will reach you. Ask who to contact if things change. You can explain your understanding in your own words and ask for further clarification.

This worksheet prepares a conversation; it cannot diagnose a condition or decide whether a problem can wait. In an emergency, contact your country’s emergency services rather than waiting for a scheduled appointment.
